Social Event Registration Form

Instructions 

  1. Read and complete the Social Event Policy and Social Event Registration Form. 
  2. The completion of this form is required for all chapter-related events where alcohol is present. The completed form must be returned to Student Activities, 219M Student Center, at least 2 days prior to the date of the event.
  3. A complete guest list must be submitted no later than 5 PM on the Thursday before the event.
